Did you attend the 2009 Summer Ball?
 We need your views.
Dear Parent/Guest
Preparations are already underway for next year’s Summer Ball at Verulam School.  
In order to ensure the Ball remains as enjoyable as ever, the Summer Ball Committee would very much appreciate your feedback on the 2009 Ball .
Primarily because of lower than expected ticket sales, in 2009  we changed the venue from a marquee to the main school hall. We also changed the band and the caterers. We would very much like your view on these changes, and how  you felt the ball compared with  previous years.
At the bottom of  this page, you will find a brief questionnaire that we hope you will find a few minutes to complete and return.
Finally, if you would like to help out with preparation for the ball, or have an idea for a new event, please do get in touch with us at summerball@verulampta.org.uk  .

Instructions

If you have Microsoft Excel:

1) Download the Excel version by clicking FEEDBACK EXCEL, and pressing SAVE (not Open) when the window pops up;

2) Fill in the form;

3) Open up an email to Simon by clicking HERE

4) Attach the filled form to the email and press send.

 

Alternately:

1) Open up a PDF version in a new window by clicking FEEDBACK PDF .

2) Print the form and fill it in;

3) Post the form to the address at the bottom of the sheet.
